How to File a Community Concern Form

Fill in the Details: The form will ask for your name, contact information, and relationship to the school (student or parent). Please fill in these details accurately.
Describe Your Concern or Recognition: In the provided text box, please describe your concern or the positive event you wish to recognize. Be as detailed as possible to help us understand the situation better.
Submit the Form: Once you have filled in all the details, click on the ‘Submit’ button.
Follow-up: Our school administration will review your submission and take appropriate action. If necessary, we may contact you for further information.
